The question that is "misanswered:"  Who started the events in questiuon at the State of the Union?

My point of view:

The accepted protocall for a Speaker introducing the President  is well established: 

"It is my distinct honor and personal previlige to introduce the President of the United States,"  a phrase used for decades. "  

Pelosi omitted "It is my distinct honor and previlige to introduce  . . . . " and with that omission,  she declared war on the President. 

Understand that this omission was a blatant and in-your-face insult of the President so,  of course, he didn't shake her hand.  The fact that Trump did not shake her extended hand hand is clear.  The belief that this was an intentional response to the Speaker's insult is in question, however.  When you watch the video of the Address,  you will note that Trump handed out the two official copies of his Address and was actually in the process of turning to audience when Pelosi extended her hand.  Further, it is clear that Trump did not see her hand before he started turning.  

It is also clear that Pelosi's stunt (tearing up the speech) was both intentional and premeditated  . . . .  a first for such an action (in the history of the nation). 

End of story.  Case closed.

As an aside:   And never forget that Romney has aligned himself with the likes of Pelosi..
